Recovered patients are now able to receive an online Teudat Machlim (Certificate of Recovery). Similarly, vaccinated persons can receive a Teudat Hitchasnut (Certificate of Vaccination). Enter your details at this site.

It is crucial that all information be entered correctly, including the phone number which appears on your Kupat Cholim account.

The process works smoothly for Israeli citizens. However, non Israelis who are Kupat Cholim members have been receiving teudot which do not display their English names or passport numbers. It goes without saying that this can potentially hamper travel plans, as in the future, proof of vaccination may be required for travel. Persons without Kupat Cholim who have been vaccinated may apply with their passport numbers. However, many passport holders are experiencing trouble in printing their teudot.

Chaim V’Chessed has reached out to government leaders and Health Ministry officials regarding these issues, and we hope to have a resolution soon.

See below letter sent by Paysach Freedman, CEO of Chaim V’Chessed to MK Yitzchak Pindrus who has been deeply involved in assisting community members in these matters.
