NEW REGISTRATION FORM – Lawrence Public Schools Kosher Meal Distribution – February 24th

Dear Parents,
The Lawrence Public Schools will continue its distribution of food to children 18 and under who live in Nassau County. To be eligible Children that attend a school currently providing any federal child nutrition program (not just those participating in NSLP) are not eligible for the Lawrence UFSD meal distribution. It cannot be stressed enough that duplication is absolutely forbidden. By signing up and picking up food you are assuring the Lawrence School District you have not picked up form any other location.
There is a new sign up form which was designed to ensure no duplication and only eligible schools may participate. For your convenience please utilize the same phone number every week.
The eligibility requirements:
·    Children 18 years and younger
·    Every child must be registered
·    You must live in Nassau County
·    You must enter the child’s name and eligible school
The intention of the expansion of the food program is that all children eat. IT is not based on financial needs.– If you meet the qualifications above, ALL children are eligible and are encouraged to sign up.
Beginning this week, boxes will be distributed based on number of eligible children.

Reminder
1)  Have a printed email response sheet with a QR code for all families for whom you are picking up. PLACE IT ON THE DASHBOARD prior to entering the driveway!
2)  YOU MUST WEAR A MASK – Wear a mask upon entering the driveway.
3)  Have a clean and clear trunk. (You may leave an empty box for the milk)

· MUST BE 18 YEARS OF AGE OR YOUNGER · MUST RESIDE IN NASSAU COUNTY
· IS ATTENDING A SCHOOL OR DAY CARE THAT DOES NOT PROVIDE FOOD THROUGH ANY GOVERNMENT PROGRAM (SSO, SSP, NSLP, SMP)
· CHILDREN WILL BE APPROVED INDIVIDUALLY
TO REGISTER YOU WILL BE REQUIRED TO:
· ENTER A VALID ZIPCODE
· ENTER EACH OF YOUR CHILDREN’S FULL NAMES, AGE AND ATTENDING SCHOOL
