As of January 4, 2021, eligible New Yorkers can get the COVID-19 vaccine at sites located throughout the city.
The following groups are now eligible for vaccine:
- All outpatient and ambulatory frontline, high-risk health care providers who provide in-person patient care or other staff in a position where they have direct contact with patients of any age. This includes hospital and community-based ambulatory care, primary care, and outpatient behavioral health service providers; phlebotomists; physical and occupational therapists; and specialty clinics, including dialysis centers.
- All frontline, high-risk public health workers who have direct contact with patients
- Nursing home and other long-term care facility residents and staff
- Health care workers at COVID-19 testing sites
- High-risk hospital and Federally Qualified Health Center staff
- Emergency medical service (EMS) frontline personnel
- Medical examiners and coroners
- Funeral workers who have direct contact with infectious material and bodily fluids
- Staff and residents in group living facilities run by the NYS Offices for People With Developmental Disabilities, Mental Health, and Addiction Services and Supports
- Urgent care providers
- Staff administering the COVID-19 vaccine
- Staff at dental practices of all types

Where to Get Vaccinated
Contact your employer to receive your vaccination if you work at or are affiliated with:
- A hospital network
- An urgent care center
- Federally Qualified Health Centers
- A congregate setting associated with the New York State (NYS) Offices for People With Developmental Disabilities, Mental Health, and Addiction Services and Supports
For all other eligible groups, including eligible health care workers not affiliated with a hospital and eligible funeral workers, first check with your employer to see if other vaccination plans have already been made. If not, you can be vaccinated at the following places:

If you are currently not eligible for vaccination, do not sign up for an appointment at this time.
Before Your Appointment
You will need to complete the NYS COVID-19 Vaccine Form (PDF) in order to get vaccinated. NYS requires the provider administering the vaccine to check that you completed the form.
In addition, you will need to bring proof of employment, such as an employee ID card, a letter from your employer or affiliated organization, or a recent pay stub.
Before you visit a site for your vaccination, keep in mind the following tips:
- Reschedule your appointment if you are not feeling well on the day of your appointment.
- Wear a face covering to your appointment. You will not be admitted to the site without one.
